29-3-11 50M DH L4-5, Free disk fragment.

A 50 year- old male, 2M of LBP and L5, S1 left leg pain. His cafl is atroply, (35/31).,LATR 2/4. SLT 40. MRI show DH L4-5 with free disk fregment move down at the L5-S1 level. I did fenestration at L4-5 first, after taking out the L4-5 free disk fragment ,Left L5 nerve have swelled, so it have been narrow. I have been not sure that there was more free disk below susch as on MRI, so I did hemilaminectomy, athough I didnt see any thing more ,it make the L5 nerve more comfortable. Infact, the last axial picture show that the left nerv was swelled. we dont to do hemilaminectomy, I am sorry about it.




29-3-11, 54F LSTL3-4-5-S1

54 year-old-female, 6m LBP & L5 left leg pain. MRI demonstrate LSS L3-4,L4-5 and HD L5-S1, but on Xray it less stenosis than MRI. Actually, after decompressing flavum ligment, I did see the compression such as MRI at L5-S1 . This case I think that I dont need compress the L3-4 .

26-3-2011 HD L4-5

36 year-old- man, 2 years with LBP radiated left leg pain. MRI show lumbar stenosis with bulge disk L4-5 on left. Non leg pain after operation.

25-3-2011, lateral recess stenosis L4-5

48year-old- man, 2 years intermitten claudication; 2 months with L. leg pain & numbness L5, LSRT 90, EHL 2/4. MRI show lateral recess stenosis on the left L4-5, I only did fernestration on the left side although we can see bilateral lumbar stenosis with bugl disk at the center.

24-3-2011 DH L4-5 18 YEAR-OLD

A 18 year- old man, 8m with LBP and R.leg pain L5, RSRL 30, R.calf muscle atrophy, REHL 2/4 I saw that there was a larg DH at rigt side L4-5, but I didnt see too large part DH such as on MRI when I operated, I think that thsi patient is tall , young. The disk is fresh, spinal canal is larg.

23-3 2011 HD L4-5

A 23year-old male, LBP and R. leg pain for 1 years, no improved with conservetive therapy. There wase a bulg disk with flavum hyperligment on right side at level L4-5.




22-3-2011, DH L4-5

A 36 year-old male, 2 years with LBP and bilateral legs pain. He was very painful. First day post-operation he was no pain, but still numbness. most L4-5 disk was damaged, although we dont see unstable on dynamic Xray, but I think that he will be unstable at level L4-5 later.

22-3-2011, DISK HERNITAED L4-5

A 26 year-old male, 10 days with LBP and left leg pain. He cant sleep. After oparating, no pain in leg, he felt very happy.
